Healthy People 2020 has featured the Integrated Health Network's Community Referral Coordinator (CRC) Program in their sharing library of "Who's Leading the Leading Health Indicators?" The work of the CRC program is working towards Healthy People's goal of improving access to comprehensive, quality health care services. The "Who's Leading the Leading Health Indicators?" Series provides an overview of LHI topics, noting the most recent data and trends, and showcasing states, communities, or organizations that are addressing the LHIs in innovative ways. Each month, Healthy People 2020 highlights a new topic area and a new community group. Successful strategies and effective data for groups that are featured in the series are distributed to approximately 30,000 public health professionals via Healthy People 2020 listservs.
Healthy People 2020 is part of the U.S. Department of Health and Human Services, and is currently working on an ambitious, yet achievable, 10-year agenda for improving the Nation's health. For three decades, Healthy People has established benchmarks and monitored progress over time to; encourage collaborations across communities and sectors, empower individuals toward making informed health decisions, and measure the impact of prevention activities.
