Principal Strategist - Alignment, Innovation, Growth
Before moving to Missouri, Jess Holmes served families as a Case Manager for community-based behavioral health organizations in Indiana. In this capacity, she learned that patients’ well-being is best achieved when the systems around them align to support their goals.
In her role as Principal Strategist at the IHN, Jess drives alignment and innovation across systems serving the safety net. Along with the IHN leadership team, she supports IHN’s P.U.L.S.E. initiatives in St. Louis and Rolla, MO. The P.U.L.S.E. Model is a tiered community intervention at the patient and the systems levels. At the patient level, Community Referral Coordinators navigate patients from a hospital setting to primary and community-based care. At the systems level, the Transitions of Care Task Forces convenes leadership across healthcare to prioritize and coordinate systems of care. In addition, Jess oversees an innovative pilot in St. Louis City called Re-Entry Community Linkages (RE-LINK). RE-LINK combines a collective impact approach with Community Health Workers to connect the silos between jails, health, and social services to better serve reentrants and reduce their likehood of returning to jail.
Jess co-chairs the Healthy Eating Active Living (HEAL) Healthcare Access workgroup focused on scaling and sustaining Community Health Workers in the St. Louis region. She serves on the Board of Directors for Missouri CASA, a statewide advocacy organization that serves children who are involved with the child welfare system. She received a Masters in Social Work from Washington University in St. Louis as a distinguished Bettie Schroth Johnson Scholar for outstanding women in social service management.
When asked why she is glad to be a part of the IHN, Jessica states, “I see the IHN’s work as unique in that it is a mission-driven organization that yields better outcomes and quality of care for patients while also being cost-effective. I believe the IHN’s success in these areas is driven by our collaborative approach and commitment to put patients first. I am very proud to have a role expanding the impact of our meaningful work.”
