At the heart of the St. Louis Integrated Health Network (IHN), we are committed to advancing health equity and improving well-being through increased access to health and social services. One of our key initiatives in this mission is the Epidemiological and Laboratory Capacity Community Health Worker (ELC CHW) Initiative, a program specifically designed to support at-risk pregnant individuals.
Understanding the ELC CHW Maternal Health Initiative
The ELC CHW Initiative is a targeted program for pregnant individuals who are experiencing social determinants of health and underlying health conditions. Our initiative is guided by four primary goals:
- Improve Access to Healthcare: We strive to increase access to healthcare for at-risk pregnant individuals with underlying health conditions or COVID-19 related illnesses.
- Increase Referrals: Our team enhances referrals to community resources, clinical services, and lifestyle programs through the efforts of Community Health Workers (CHWs).
- Improve Transition to Care: We facilitate smoother transitions to care for expectant mothers.
- Decrease Adverse Birth Outcomes: Our aim is to reduce the incidence of adverse birth outcomes through targeted interventions and support.
