Returning home from incarceration should not mean losing access to health care. The Saint Louis Integrated Health Network (IHN), in partnership with the Transitions Clinic Network, is bringing the first Transitions Clinic program in Missouri to Key Clinic in St. Louis. The program connects people returning to the community with primary care and practical support to help them stay connected to care.

Care and Support for the Transition Home

The program is based at Key Clinic, a walk-in primary care clinic operated by DOORWAYS in the JeffVanderLou neighborhood. It is designed for people returning from incarceration who need a reliable connection to health care. The initial St. Louis pilot prioritizes adults ages 18 to 45 in St. Louis City and St. Louis County, with a goal of enrolling approximately 100 people in its first 12 months.

A Connection to Primary Care

Build an ongoing relationship with a care team at Key Clinic, rather than having to navigate health needs alone after release.

Support From Someone Who Understands

Community health workers with lived experience of incarceration help participants navigate appointments and the challenges of reentry.

Help Beyond the Clinic

Participants can get help connecting with resources for transportation, housing, insurance, identification, employment and behavioral health care.

How the Program Works

IHN is working with corrections, probation and parole, hospital discharge and community reentry partners to create ways to refer people to the program. After a referral, a community health worker can help schedule an appointment, coordinate transportation assistance and provide reminders. The goal is to help each participant establish a primary care home and stay connected to care over time.

“People returning home from incarceration are often navigating multiple systems at once, and health care can easily become one of the things that falls through the cracks. This clinic is designed to change that by creating a consistent connection to care and pairing primary care with support from a Community Health Worker who understands the reentry experience firsthand.”
